A 62-year-old man with a history of normal health visits his physician for a checkup. During his physical examination, it is revealed that his blood pressure is 168/96 mm Hg. Further tests indicate that he has renovascular hypertension as a result of a stenosis in the left kidney. The answer is E.
Hypertension can occur when the artery of one kidney is stenosed. The decrease in renal blood pressure resulting from the stenosis causes the kidney to secrete large amounts of renin. The renin causes angiotensin II to be formed in the blood. Angiotensin II causes peripheral vasoconstriction and thus raises total peripheral resistance (TPR) and blood pressure. Angiotensin II has a powerful effect on the adrenal glands to cause aldosterone release. The aldosterone causes retention of salt and water, exacerbating the hypertension.
