Dear friends,
those of you who failed Oct 09/earlier like myself, lets get together and write our respectice case scenarios esp st 5.
my exam in oct 09:
STATION 5
1 amenorrea + headache
diagnosis was PROLACTINOMA--SHE HAD GALACTORREA +
2 DM + HYPOTHYROIDISM + POSTURAL HYPOTENSION(ADDISONS)
POLYENDOCRINE SYNDROME

Thank u Jamaldana. I heard that there are some books which are relevant for making summaries, or making paces easier, or about the communication ethics...etc. I will join the pastest also. But I need to read some books now. Thanks alot.
Anybody can we together discuss and be a study partner via this website?

Read up on clinical skills and learn the history taking and investigation and management mnemonics from the paces clinical skills book
Then practice on patients and do history and communications practice with patients and colleagues

Dear all,
I have a little advice regarding the new station five:
don't panic, this is the only station that you have to score something even if you did unwell.
you have 14 points in this station for each examiner, this is 28 points
if you maintain pateint wellfare you'll get 4
if you address pt's concerns this is another 4
if you took a very good history again this is 4
if you explain your plan to pt in a good manners this 4 or even 8 if your dx is right
these are 16-20 points without even touching the pt
I passed my last attempt and I got 13+13 in first case and 10+10 in the second case in which I had a large argument with the examiner about the judgement wether I should start asperin in a TIA pt with hx of peptic ulcer dis(no bleeding) last year, and I thought that they will tear me to shreds, surprisingly they gave me unsatisfactory mark in judgement and examination only and I got 10/14. while overall I got 158/172
so don't panic, because new station 5 should be the last thing you should worry about.

Read the clinical skills book from cover to cover - minimum 3 times
Practice examination and communication with patients and colleagues
Learn all the causes of clinical features and memorise them
Learn how to put the clinical features together to make a diagnosis
DO NOT WASTE TIME MEMORISING CASES

iam sorry i disagree with the guest,look i did a paces course also in london which realy helped me,the paces exam does present certain case which have a high chance of comming in exam i mean the examining staations,like cards test as ar ms mr vsd afib artifical heart valve like aortic and mitral,cabg pacemakers .so you must learn these as best as you can practice good exam tecnique,abdo common cases you must learn are liver cirrohosis,hepatosplenomegaly ascites liver transplant kidney transplant ibd crohns
station 5 is like a lottery again common cases like ra thyroid goitre,complaints like vision problems,dvt sob,cough and hemoptysis skin conditions like psoriasis neurofibromatosis,diabetic neuropathy ank spond
neuro common cases you must prepare are charcot marie tooth periheral neuropathy,cva cerebellar syndrom like ms ino,nystagmus,spastic ,pupillary abnormalitiesparapareisis,old polio,parkinsons,again read these well
resp cases commonly tested pulmonary fibrosis,cfa,copd old tb thoracotomy pleural effusion,lung ca.
history taking and ethics again please see the hoole book it is extremly capable book
in the end remember paces is a very stressfull exam you will be rushed paces have the highest and toughest passing standar its about 80 percent,ie you need to score 80 percent or greater to pass,there are 11 examiners ,.

In my opinion of PACES exam, see as many patients as possible to tackle the clinical stations, use every opportunity when seeing patients on MAU or on ward think about what stations it could be and presents to your seniors or those who have passed their PACES exam. Find those seniors who are "hawks" to question you left, right and centre. Use the opportunity to teach medical students so that you can explain the signs available on the patients to them.
Practice taking a good solid focal history from patients(sometimes it might not be feasible for acute medical patients), practice communication skills
My strong advice is NOT to read too much to have temptation to regurgitate physical signs from books
I find it useful to go for courses 1 a month before the exam and if possible another one 2weeks before the exam especially those courses that offer mock exams. My recommended good PACES courses are Hammersmith PACES course in London and PASTEST PACES course in Manchester, didn't get the chance to attend courses with mock exams!!
Remember the PACES exam is an exam to test clinical examination skills and the ability to communicate with consultants at Membership level.
Last but not least, practice makes perfect, good luck in the upcoming exam.

| 2 n half months enough for paces preparation ?how should we start ? does it means no need to buy any book ?just use the basic clinical books we had used in our house job n training period?

Success in PACES by Douglas McDonald/Philip Kelly (2nd Edition) and Ryder/Mir are good for History taking and communication skills.
Tim Hall is good for Histoy taking as well.
Stephen Hoole (small book) for putting it all together and Oxford Handbook for reading text.
You will need a partner to practice communication skills and history taking. You need to practice clinical examination methods and presenting the case.
Also need to learn how to do fundoscopy.
I believe PACES preparation courses in the UK are very useful for orientation of the exam and also for quick learning. The ones with mock exam are even more useful.
Station 5 cases need to be practiced very carefully as well. Just think you are seeing a patient in OPD and proceed in the best way you can.
How much time you need for preparation depends how much clinical experience you already have and how much time you can give for preparation in this 2 and 1/2 months.

| There is a very good PACES course conducted in India at Trivandrum for 4 days called EXCELPACES. The teachers are top class and you will be really grilled and trained regarding examination techniques and presentation. They have a mock examination and the cost is half of that of a UK PACES course. you will greatly benefit from this course. Book early as they are much in demand. Their website goes by the same name excelpaces
