MEDICAL specialists are profiting from referring patients to radiology businesses they part-own, in a practice the Federal Government has asked the Australian Competition and Consumer Commission to investigate.
&lt;br /&gt;

&lt;br /&gt;
According to the Royal Australian &amp;amp; New Zealand College of Radiologists, doctors are forming joint ventures, in which 20 to 30 of them enter into business with radiologists that they refer to, splitting the profits.
&lt;br /&gt;

&lt;br /&gt;
But these groups risk draining the Medicare purse and adding a health risk if patients are radiated too often, said Melbourne radiologist Dr Matthew Andrews, who is president-elect of RANZCR.
&lt;br /&gt;

&lt;br /&gt;
Medicare is also investigating the groups, which have sprung up in WA, Queensland and NSW in the past 18 months due to what RANZCR calls a ''loophole'' in federal law.
&lt;br /&gt;

&lt;br /&gt;
The college says they are poised to expand again - possibly to Victoria - once a new Medicare rebate for CT scans comes in to effect next month.
&lt;br /&gt;

&lt;br /&gt;
''In practice it may be above board but there's always the possibility that things could be tainted,'' Dr Andrews said. ''If the doctor has an interest in the imaging practice, first up the decision to perform the imaging could be influenced … [and it] has the potential to influence the doctor saying where the patient should go.''
&lt;br /&gt;

&lt;br /&gt;
Current RANZCR president Mark Khangure said experience in the US with ''vertical integration'' of referrers and service providers in radiology had led to a 10 to 15 per cent increase in the number of scans ordered.
&lt;br /&gt;

&lt;br /&gt;
''You end up with a situation of overservicing,'' Professor Khangure said.
&lt;br /&gt;

&lt;br /&gt;
An imaging specialist who did not want to be named said small practices were losing a lot of business to the new groups, adding that patients had told him the specialists did not declare their interest in the radiology business to which they were being referred.
&lt;br /&gt;

&lt;br /&gt;
Last year a new federal law came into effect banning direct or indirect kickbacks from radiologists to referring specialists. However, RANZCR says a glaring loophole in the law allows joint-venture type arrangements.
&lt;br /&gt;

&lt;br /&gt;
Earlier this year the Government asked Medicare to investigate several such practices, to see if they were complying with the law. And it recently referred one group to the ACCC.
&lt;br /&gt;

&lt;br /&gt;
Depending on the ACCC's report, the Government may rewrite its legislation or begin a wider review of referral practices.
&lt;br /&gt;

&lt;br /&gt;
Health Minister Nicola Roxon said any rorting of Medicare was unacceptable.
&lt;br /&gt;

&lt;br /&gt;
''Legislation was introduced last year by the Rudd Government to prohibit inappropriate commercial relationships between requesters and providers of pathology and diagnostic imaging services,'' she said. ''Ultimately, the question of whether or not any arrangements are in breach of the legislation is a matter for the courts to decide.''
&lt;br /&gt;

&lt;br /&gt;
The Age contacted the WA-based radiology company Envision yesterday. Chief executive Julie Carr confirmed it was 40 per cent owned by about 20 specialists, but said Envision had legal advice that the interests of the specialists were ''permitted benefits'' under the law because their return on investment was not based on referral patterns.
&lt;br /&gt;

&lt;br /&gt;
She also denied that the arrangement increased the rate of radiology referrals, saying the ''monitoring role of Medicare'' would catch any doctor behaving unethically.
&lt;br /&gt;

&lt;br /&gt;

gave paces yesterday in the uk.
&lt;br /&gt;
my cases
&lt;br /&gt;
renal transplant
&lt;br /&gt;
pul fibrosis with clubbing
&lt;br /&gt;
aortic stenosis
&lt;br /&gt;
charcot marie tooth
&lt;br /&gt;
history,young diabetic with poor control
&lt;br /&gt;
ethics adverse drug complication family to counsel
&lt;br /&gt;
station 5
&lt;br /&gt;
ra no joint deformities
&lt;br /&gt;
 a chap smoker on ramipril with cough and hemoptysis
&lt;br /&gt;

&lt;br /&gt;

&lt;br /&gt;
my thoughts
&lt;br /&gt;
kudos to rcp and the examiners
&lt;br /&gt;
 a tough but fair exam
&lt;br /&gt;
very pleasant examiners
&lt;br /&gt;
TIME PRESSURE ,NOT MUCH TIME FOR CASES EXAM STATIONS,LIKE STATION 5 WATCH YOUR TIME VERY CLOSELY
&lt;br /&gt;
I THINK THE EXAM IS RUSHED,HAVING SAID THAT THEY TEST YOU HOW YOU WILL PERFORM UNDER STRESSFULL SITUATIONS.
&lt;br /&gt;
IT IS NOT FUN WHEN YOU ARE WATCHED BY TWO EXAMINERS WHO ARE EIMeMENT PHYCISIANS
&lt;br /&gt;
BODY LANGUAGE SPEAKS FOR YOU.
&lt;br /&gt;
WHAT I DID BAD
&lt;br /&gt;
messed up the resp and abd stations,got diagnosis and exam fairly right but got confused by examiner questions they were a particularly hard pair rest i did fairly well i hope i pass but who knows,be ready to defend your findings do not lie  say you dont know or did find it,do not confabulate or invent signs not present.
&lt;br /&gt;

&lt;br /&gt;
my advise for future candidates
&lt;br /&gt;
go on a course,it is a must.i did pastest,which was a great course,if i pass it will be due to it,if i fail my own fault.
&lt;br /&gt;
practice practice and practice under exam conditions as much as possible
&lt;br /&gt;
i think best to revise from the hoole small book that is more than enough.
&lt;br /&gt;
be calm even doing badly,nervousness is certain but stay calm
&lt;br /&gt;
read instructions carefully.
&lt;br /&gt;
always thank the patients and the examiners in the end for allowing you to appear.
&lt;br /&gt;
respect patients and get their consent before each exam.
&lt;br /&gt;
examiners will not give any feedback either good or bad,they may get upset if you say something very stupid or act badly,remember it is formal and professional exam of the highest international standing ,dress smartly and conservatively.
&lt;br /&gt;

The answer is C. The history given above is a classic presentation of acute post-infectious cerebellar ataxia. 25% of children with this disorder have a history of varicella infection within 1 month prior to the onset of the disorder and 5% of children have a previous history of varicella vaccination. The onset of acute cerebellar ataxia is usually explosive, often with the child awakening from sleep with maximal symptoms of ataxia and nystagmus. In any child with acute ataxia, it is important to obtain a drug screen and a careful history of any possible toxic exposures since poisoning and acute cerebellar ataxia account for most of the cases of acute ataxia in children. Resolution of the symptoms occurs in most children over a period of weeks to months.
&lt;br /&gt;

&lt;br /&gt;
Concomitant diarrhea (choice A) is not relevant to this presentation. Although there are forms of Guillain-Barre syndrome that have ataxia as a prominent component, this child does not have the other symptoms associated with these variants of GBS (ophthalmoplegia, depressed reflexes), and Campylobacter diarrhea usually precedes the onset of GBS.
&lt;br /&gt;

&lt;br /&gt;
While an intracranial tumor is always of concern in a child with neurologic symptoms, the history above is atypical for tumors, which usually present more indolently. The history of a cousin with a brain tumor does not increase the likelihood that this child's symptoms are due to an intracranial mass (choice B).
&lt;br /&gt;

&lt;br /&gt;
Pseudoataxia (choice D) is a syndrome of recurrent bouts of ataxia that are actually atypical seizures on EEG. The attacks are similar in timing to seizures and there may be a postictal state. They are not likely to persist over days as in this child.
&lt;br /&gt;

&lt;br /&gt;
Recent streptococcal pharyngitis (choice E) is of importance in movement disorders associated with acute rheumatic fever, such as Sydenham chorea, but does not predispose to any known form of ataxia.&lt;/span&gt;&lt;br /&gt;
