abscence of callus is diagnostic of non-union.
    causes of non-union
    1. malignacy, infection
    2. distraction of both ends by muscle
    3. avascular fragment
    4. comminuted fractures specially with infection
    5. a fracture in a bone that has stopped to grow
Considering the options I'd go for malnutrition.
